The Eastside Immunization Clinic (for Vaccines and Immunization Records) is located at 210 N. Mel Waiters Way (formerly 210 Rio Grande) San Antonio, TX 78202. They are available by appointment ONLY Monday through Friday from 8am to 3pm. The cost for immunization records is $5.00.

Records may also be obtained through the Texas Immunization Registry.
Individuals who did not receive a vaccination card at the time of their vaccination should reach out to the provider where they received their vaccine. They may also contact the Eastside Immunization Clinic.
